- Conduct new client assessments and care plan development.
- Perform supervisory and quality assurance visits in clients' homes.
- Complete required documentation and compliance activities for:
- Franklin County Senior Options (FCSO)
- Veterans Community Care programs
- Long-Term Care Insurance clients
- Private Pay clients
- Perform 60-day and 90-day supervisory visits as required.
- Evaluate client needs and recommend appropriate services.
- Provide caregiver orientation, skills validation, and return demonstrations.
- Conduct competency evaluations for caregivers and home health aides.
- Review caregiver documentation for accuracy and compliance.
- Collaborate with office staff, case managers, discharge planners, and referral sources.
- Participate in quality improvement and risk management activities.
- Assist with incident investigations and corrective action plans when needed.
- Provide clinical guidance and support to caregivers and office staff.
- Current Ohio RN License in good standing.
- Minimum of 2 years nursing experience preferred.
- Home care, hospice, senior care, or case management experience preferred.
- Knowledge of Franklin County Senior Options and Veterans programs preferred.
- Strong assessment, documentation, and communication skills.
- Ability to work independently and manage multiple priorities.
- Reliable transportation, valid driver's license, and proof of insurance.
- Proficiency with electronic documentation systems.
